To request a ride, call the Transportation Office Monday through Friday between 8am and 4pm as soon as you know you have a scheduled appointment.   920-386-3832 or 1-800-924-6407  Messages can be left after hours.

We need the following information to complete a ride request

• 3 day notice prior to appointment 

• Name, Address, Phone Number 

• If you are under or over age 55 

• The date, exact time and location (city and street of your destination) 

• How long is your appointment? 

• Will you need a ride home? 

• Do you have Medical Assistance or any other payment source? 

Vans with wheelchair lifts and volunteer drivers operate five days per week, Monday thru Friday.  Volunteer drivers receive NO PAY for their time or car expenses. They are only reimbursed for the cost of gas, parking, etc.
