The Dodge County Sheriff's Department has posted several forms
which may be of assistance to the public in addition to the
Deputies. The forms listed below may be down loaded and then
printed. You may be asked by Deputies to print a form from this page
and mail it to the Sheriff's Department. It will make dealing with
paperwork more streamlined and efficient.
If asked by a Deputy to complete a form, please do so and mail it
or drop it off in person to:

Dodge
County Victim Information
The Dodge County Victim Information
sheet is given to victims of crimes. It contains much needed
information related to victim's rights, crime victim compensation,
restitution, etc. Important phone numbers are also listed on this
form.
No
Consent Form
The No Consent form is signed by victims of
crimes saying they did not give consent for the crime to occur. This
form is needed for successful prosecution of charged individuals.
The form has three parts:
- Burglary
- Theft/Criminal Damage to Property/Operate without owners
consent (stolen autos)
- Battery
When asked to do so, please fill out the
appropriate portion of the form. Please be a detailed as possible.
If you have questions, please contact the Deputy who asked you to
complete the form.
Traffic
Accident Statement Form
The traffic accident statement form
is used by investigators to determine what occurred before, during
and after a traffic crash.
When asked to complete, please fill
out the form as thoroughly as possible. This is a two page form so
please complete both pages.
Voluntary
Statement Form
The voluntary statement form is used by
victims, witnesses and other involved people. When asked to
complete, please fill out the form in as much detail as possible.
Please print and complete as many pages as necessary.
NCIC
Stolen Property Worksheet Form
The National Crime Information
Center (NCIC) requires that this form be completed and signed by the
owner of an item that was stolen. Once this form is completed, the
item may be placed into the NCIC computer. Once the item is entered,
if the item is queried by a law enforcement agency, the item will be
shown as stolen. This query can be done nation wide.
When asked
to complete, please print this form and fill in all the blanks
possible and sign the bottom.
Authorization
for Disclosure of Health Information
The Authorization for
disclosure of health information form must be signed before the
Sheriff's Department can obtain such records. When asked, please
fill the form out completely and forward it to the Sheriff's
Department.
Community
Program/Appearance Request
The Dodge County Sheriff's
Department does frequent talks and demonstrations to public groups.
If you would like the Sheriff's Department to appear for your
organization, club or school, please down load this form and
complete. The form should be sent to the Sheriff's Department and
someone will contact you.
Community
Program/Appearance Evaluation
The community
program/appearance evaluation form is used by the Sheriff's
Department to evaluate the effectiveness of the programs.
Information obtained from the evaluations is used to improve the
quality of the presentations. We request that the form be completed
and forwarded to the Sheriff's Department after a presentation.
Business
Information Form
The business information form is completed
by business owners and forwarded to the Sheriff's Department. The
Sheriff's Department uses the information obtained to contact key
holders in event of an emergency. Having this information will make
it much easier and faster to contact business owners. Please
complete this form and forward it to the Sheriff's Department.
GPS
Application
GPS (also known as electronic monitoring or the
"bracelet"), allows an inmate to reside in his/her home while
serving a sentence. The inmate wears an ankle bracelet that monitors
his/her location. Anytime the inmate is outside of the home must be
previously approved by a Programs Officer. This application may be
completed after sentencing and dropped off at the main lobby or
mailed to:

Huber
Privilege Transfer Request
If the judge grants transfer
privileges and the judgment states transfer privileges to a specific
county to be served on electronic monitoring, a Huber Privilege
Transfer Request may be completed and dropped off at the main lobby
or mailed to:

This form is only to be used when sentenced in Dodge
County. If someone is sentenced in another county and wants to
transfer to Dodge County, he/she must contact the sentencing county
and request a transfer from that county. Transfers to Dodge County
cannot be initiated by the person who was sentenced.
GPS
Work Schedule
When an inmate is serving time on GPS, the GPS
Work Schedule must be completed and turned in at the main
lobby.
Inmate
Visitation Form
If you wish to be added to an Inmate’s visiting list,
complete this form. Send this form to the inmate so that they can
submit it. You should call before you come to visit to verify that
you have been approved. All information must be accurate.
