In-Person Application Requirements

 

To obtain copies of vital records in person you will need the following identification:  Acceptable Forms of Identification (PDF 27KB)

 

We accept cash, check and money orders for payment in the office.  We do not accept debit cards or credit cards.  If you wish to pay by credit card, you will need to make your request online.  See the link below.

Birth, Marriage and Death Certificates Through the Mail

 

To request a copy of a birth, marriage or death certificate through the mail, please print the appropriate application form from the list below and follow the instructions on the application.  Mail the application to the address indicated on the form along with a photocopy of the applicant’s current valid photo ID.

 

Application forms for birth, marriage and death certificates are available for printing or downloading to your computer.  These forms are available in the Adobe® Acrobat format.  To view a document in this format, you must have the free Adobe Acrobat Reader installed on your computer.  If you do not have Acrobat Reader, it is downloadable for free directly from Adobe's web site or by clicking on the link at the bottom of the page.

Genealogical Research

 

Research birth, death and marriage records from 1877 to present.

 

Our records were destroyed by fire in September 1877.  We have very few records that predate the fire.

 

Genealogical research hours are from 8:00 AM to 4:30 PM.  No appointment is necessary.

 

All researchers must be at least 12 years of age and produce a valid ID.
