The Department of Human Services and Health Department provides confidential comprehensive mental health services and substance abuse services to county residents of all ages. 

 

General Information

 

Eligibility:   To be eligible for ongoing services, an individual must be a resident of Dodge County. 

 

Fee Information:   Costs are covered by a variety of sources which may include Medicaid, Medicare, private insurance or an HMO, and patient fees, which may be on a sliding scale fee.  This is determined through the billing department at the time of the first appointment.  Patients who are covered under a medical plan that does not have a contractual arrangement with Dodge County Human Services, will be referred to their plan provider to determine the alternatives to being seen at this agency. 

 

Information and Referral:   The clinical services intake worker is available from 8:00 AM to 4:30 PM Monday through Friday to answer questions and help you find the right services to meet your needs.  Call (920) 386-3500. 

 

Crisis Intervention:   During regular business hours, crisis calls can be referred to the intake worker at the number above.  After hours and on weekends, the on-call worker can be contacted through the Sheriff’s Department and will respond to your initial request for contact. 

 

Services Provided

 

Outpatient Clinic

 

The Outpatient Clinic provides mental health and alcohol and drug abuse services to individuals of all ages.  The following services are included:

  • Initial assessment to include a social history and diagnostic interview to determine services needs. 

  • Individual, couple, and family therapy.

  • Psychiatric evaluation and medication management.

  • Case management services.

  • Assessment for inpatient mental health services such as psychiatric hospitalization with a need for stabilization of symptoms, adjustment of medications, and return to community safety is indicated. 

  • Alcohol and drug abuse treatment.

  • Intoxicated Driver Program Assessments. Total Cost $210. Must show in person to schedule appointment. $50 deposit required at that time. $160 due at time of appointment. Location: 199 Home Road.  Call: 920-386-3500 for additional information.

  • Counseling for addiction or abuse.

  • Assessment for contractual services such as medical detoxification and referral to halfway house/residential treatment.
