As part of
the Child and Adolescent Services Unit, there are two case
managers that work with children with disabilities and their
families. They work with clients to obtain services through
Children’s Long Term Support (CLTS), Family Support, and
also provide information referral services for families that
have children with disabilities.
Children’s Long Term Support (CLTS)
The
Children’s Long Term Support waiver serves children under
the age of 18 who have a developmental disability, physical
disability and those who have a severe emotional
disturbance. To qualify for CLTS children must have
substantial functional limitations in at least one of the
following areas:
Self care,
receptive/expressive language, learning, mobility, self
direction, capacity for independent living, and economic
self sufficiency. In addition to this, children ages 0-3
may qualify for early intervention services if they have
significant delays in one or more of the following areas of
development: cognition, physical including vision and
hearing, communication, social and emotional, and adaptation
including self help skills. Children who qualify and are
granted a CLTS slot will receive medical assistance funding
for community alternatives to institutional placement.
Family
Support
The Family
Support Program (FSP) funds are used by families of children
with severe disabilities to purchase allowable goods or
services not funded through other services that will enable
the children to reside with his/her parents, reduce stress
in the family, and avoid out-of-home placement. Family
Support funds such things as: respite, durable goods,
equipment and supplies, mentors, daily living skills
trainers, sensory items, therapeutic resources, special
needs activity programs, summer program and camps, and
architectural modifications.
Birth to
Three
Birth to
Three is a statewide program of services for children ages
0-3 who are significantly delayed developmentally or have a
physical or mental condition which is likely to result in
delayed development. Birth to Three programming provides
screening, evaluation and intervention services. Areas of
delay may include: cognitive development, physical
development (fine motor, gross motor, or vision/hearing),
speech, language, and communication development, social and
emotional development and adaptive behaviors in self help
development. Dodge County’s Birth to Three program is
contracted to Green Valley Enterprises with the County
having oversight of the contract. Green Valley now provides
administration and service coordination for families that
meet the criteria to participate in the Birth to Three
program.
