No
prospective adult family home previously denied a
certification or whose certification was revoked may
reapply for certification within two years after the
effective date of the notice of denial or revocation
unless the prospective adult family home sponsor documents
to the satisfaction of Dodge County Human Services and
Health Department that the condition which caused the
denial or revocation has been corrected. Potential adult
family home sponsors need to participate in several
interviews in the prospective adult family home. The
purpose of these interviews is to see if the home, as well
as the prospective sponsors satisfy the State and Agency
requirements and are able to meet the needs of the adults
available for future placement.

A physical
examination is required. The prospective adult family
home sponsors and all household members must provide a
written statement from their doctor on Dodge County
provided forms stating that they are free from diseases
harmful to adults and that they are physically and
emotionally able to care for special needs adults.

Prospective adult family home sponsors and any children
over the age of 12 that reside in the home must agree to
undergo a criminal background check. The applicant and
any children over the age twelve residing in the home
cannot possess criminal charges that may have an impact on
caring for vulnerable adults in a safe and appropriate
manner. The prospective adult family home sponsor and
every substitute provider shall be at least eighteen years
of age and shall be physically, emotionally, and mentally
capable of providing responsible adult family home care.

The adult
family home sponsors and any substitute providers shall be
persons who are responsible, mature and of reputable
character, who exercises sound judgment and display the
capacity to successfully care for disabled adults.

The
prospective adult family home sponsors shall present
evidence of having or having access to sufficient
financial reserves to meet the needs of all residents and
of all members of the household for whom this sponsor is
financially responsible for. Furthermore, the prospective
sponsors must be able to ensure that the home will
continue to function adequately for a period of at least
30 days without receiving payment for the care of any
resident.

Prospective adult family home sponsors must furnish proof
of their automobile liability insurance as well as their
homeowner’s and/or renter’s insurance.
The
prospective adult family home shall be physically located so
that residents can easily get to community activities and
supportive services by walking or by means of private or
public transportation. Furthermore, the prospective
sponsors agree to ensure that the adult resident receives
the assistance necessary in order to enable them to get to
these activities and services. The prospective adult family
home sponsors shall be enrolled in or have completed eight
hours of department approved training related to the health,
safety, welfare, rights and treatment of residents during
the first year after certification. Furthermore, the
prospective sponsors shall complete six hours of agency
approved training related to the health, safety, welfare,
rights and treatment of residents on an annual basis. |
