Do
I receive a grant for being an Adult Family Home Sponsor?
The Dodge
County Human Services and Health Department along with the
Adult Family Home will establish a base rate for each adult
placed in your home. This base rate will be determined by
expenses associated with your home. In addition, the
individual needs of the adult resident and the degree of
services provided by the adult home sponsor that may
increase the overall rate. This is known as a supplemental
rate. All adults placed in an adult family home will have a
rate assessment to determine the individual’s needs. This
rate assessment will be re-evaluated on a yearly basis.
In some
cases, residences may not have income adequate to fund the
determined rate. When this is the case, other funding may
be available through Dodge County Human Services and Health
Department. You will be caring for individuals, age 18 and
over, who are unable for a variety of reasons to care for
themselves.
2. What
type of residents would be placed with me?
Adults who
are unable to care for themselves will have needs that will
fall into one or more of the following target groups:

Frail
elderly and elderly with Alzheimer’s disease and other
dementias

Developmentally disabled adults or adults with a
controlled mental or emotional disorder

Physically
handicapped adults

Chemically
dependent adults
3. What services will I provide?
As an adult family home, you will provide safety,
supervision, and stability for adults with special needs.
Adult family homes need to be accepting, supportive, and
caring homes which foster community connections. They
provide meaningful, long term, and mutually respectful
relationships. The Dodge County Human Services and Health
and the adult family home will work together to develop and
maintain the skills and independence of the individual in a
supportive home environment. Adult family homes encourage
independence by providing safety while promoting individual
growth.

What qualities are required to be certified?
A desire to share your home, time, and companionship, the
ability to encourage independence and teach daily living
skills, flexibility and responsiveness, the ability to work
with community resources, family members, and social
workers/case managers, personal and financial stability,
secure family life, and a home or apartment which you may
own or rent that meets the requirements of the Adult Family
Home Standards.
5. Will I receive training as part of the
Dodge County Adult Family Home Program?
Yes, all sponsors shall complete an 8 hour adult family home
orientation during the first year of their initial
certification. The orientation program will cover issues
involving the health, safety, welfare, rights and/or
treatment of the adult residents. The initial orientation
will also cover information pertinent to the target that you
wish to serve.
Beginning the second year of certification, each sponsor
shall complete at least six hours of Dodge County Human
Services and Health Department approved training. All
approved training must relate to the health, safety,
welfare, rights, and/or treatment of the adult residents. |
