The Adult Family Home Program of Dodge County
was initiated in the early 1970's with the intent to provide
an alternative to institutionalization for persons who were
frail elderly, physically disabled, developmentally
disabled, mentally ill and/or chemically dependent. The
main components of the program include: recruitment and
training of sponsors, matching prospective adults with
potential sponsors, and on-going support for the sponsors
and adults placed while the living arrangement exists.
The Adult Family Home Program serves adults
(age 18 and above) who, because of physical, mental or
emotional limitations, are unable to continue independent
functioning in the community, but who do not require
institutional care. Such a person needs the support and
security of family living that an Adult Family Home Sponsor
can provide.
All individuals have basic human needs (i.e.,
physical, emotional, spiritual) which they are capable of
achieving to a certain degree, if they are provided with the
appropriate amount of supportive services. Thus, it is our
goal, together with the Adult Family Home, to help each
individual meet these needs to the degree that they are
capable by providing services through Adult Family Home
Care. The sponsor helps the adult resident by including
him/her in home and family living activities.
Adult Family
Home Care gives those unable to live alone an opportunity to
receive needed supervision and services in a family
setting. We want persons to maintain a level of independent
functioning consistent with their abilities, to develop or
maintain a meaningful level of community participation, and
any other goal(s) or objectives specific to their individual
service plan.
