Did You Know?...Clearview provides long term care to people with dementia, developmental disabilities, chronic mental illnesses, and the aged.

 Did You Know?...Clearview provides short term rehabilitative care for persons recovering from traumatic brain injuries, surgeries, and other acute medical conditions.

Did You Know?...A physical restraint is defined as “any manual method or physical or mechanical device, material or   equipment attached or adjacent to the resident’s body that the individual cannot remove easily which restricts freedom  of movement or normal access to one’s body”.

Did You Know?...Whenever it is identified that a resident  may benefit from the use of a restraint, either to promote   more independent functioning or to help maintain their safety, the following is protocol:

• Alternatives must be tried prior to the use of a restraint. If a restraint is deemed necessary one must always look at the least restrictive option.
• A comprehensive assessment is completed.
• Informed consent is obtained from the resident or their legal representative, and a physician’s order is obtained before a restraint can be applied.
• After the restraint is applied, the need for it is monitored on a routine basis.

 Did You Know?...That a side rail on a bed is considered a restraint, even if it is used to promote resident independence in bed mobility or per their request for feelings of additional security

 Did You Know?...The following are examples of restraints:

• trunk restraint is any device, equipment or material that the resident cannot easily remove (i.e. a seat belt in a wheelchair used to promote better positioning and body alignment)
• limb restraint includes any device, equipment or material that the resident cannot easily remove, that restricts movement of any part of an upper extremity (ie. hand or arm) or lower extremity (ie. foot or leg)
• a chair or  device placed in a chair to prevent a person from rising without necessary assistance (ie. a lap board or cushion or reclining wheelchair)

 Did You Know?...Clearview’s mission is to provide compassionate individualized care for all people and monitors the care delivered by: ·        compliance rounds

·        individual resident care plan reviews in which the resident and family are invited to participate

·        Quality Assurance meetings

·        resident council

 Did You Know?...Since 2003 Clearview has been working with a national quality improvement organization, known as Metastar, in an effort to rise above industry care standards. 

 Did You Know?...

• Restraint usage at Clearview South is less than half of what it was in 2003 and is below the national average of 5.1%.
• Restraint usage at Clearview North is less than half of what it was in 2003 and Clearview continues to meet quarterly reduction goals set in partnership with Metastar.
